Journey’s End Refugee Services, Inc. is a refugee resettlement and immigration services provider for the Western New York Region. Journey’s End works to assist newly arriving refugees to find gainful employment and avoid dependency on social services.
TITLE: INTENSIVE CASE MANAGER
Supervisor: Social Worker
Status: Full-time, hourly, non-exempt, professional starting at $17 per hour

Job Description
The Intensive Case Manager’s primary responsibility is to maintain a caseload of high needs clients and work with them towards achieving independence and self-sufficiency. Duties include managing all healthcare-related tasks for clients such as medical referrals, appointment scheduling, and healthcare orientation.
The Intensive Case Manager must exemplify Journey’s End’s core values in their work. These values include intercultural responsiveness, integrity, person-centered, empowerment, and exemplary service.
Job Responsibilities
Maintain caseload of clients and provide services to meet the needs of these clients according to guidelines. This includes intensive cases that may require specific referrals and additional attention.
Maintain clients' case files by creating and reviewing case notes; logging events and progress; follow program guidelines to satisfy case file requirements.
Work with other departments at Journey’s End through client referral process
Complete intake process with newly referred clients
Provide culturally competent resettlement services to a diverse group of clients to support the transition to self-sufficiency
Work closely with partners to ensure clients are connected with all services needed
Make medical referrals and transportation arrangements as needed
Attend weekly resettlement department meeting.
Other duties as assigned.
Refugee Health Promotion (RHP)
Make medical referrals and transportation arrangements for all non-PC cases.
Schedule dental appointments and arrange for transportation for all clients.
Conduct healthcare orientations for all new arrivals.
Conduct cultural sensitivity trainings for new medical partners.
Complete the RHP service form; provide reports to Grants department on time.
Other duties as assigned.
Qualifications
Bachelor’s degree preferred in human services such as counseling, social work or related field.
At least one year of experience in direct client service, health care or health insurance navigation, and/or medical or mental health case management.
Experience working with diverse populations
Ability to accurately and clearly document work through written case notes
Proficient with Microsoft Office
A passion for resolving challenging problems with creative solutions
Strong communication skills and ability to work as a member of a team
Ability to adapt to diverse and challenging scenarios involving clients, partners, staff, and service providers
Demonstrated ability to successfully resolve challenging problems through use of critical thinking skills
Compassionate personality and general interest in human services
Ability to appropriately manage stress and respond professionally to partners and respectfully to clients
Valid driver’s license and access to insured vehicle
